Intravenous Nesiritide vs Nitroglycerin for Treatment of Decompensated Congestive Heart Failure A Randomized Controlled Trial

چکیده
HEART FAILURE OCCURS IN 4.7 million persons living in the United States, and is the discharge diagnosis in approximately 3.5 million hospitalizations annually. Hospitalizations account for 60% of health care expenditures for heart failure. Despite its enormous human and economic burden, no new intravenous agents for acutely decompensated congestive heart fa i lure (CHF) have been approved for use in the United States in more than a decade. Furthermore, the rapid relief of symptoms without significant complications or adverse effects of drug therapy have not been addressed previously in patients hospitalized with heart failure. There is increasing recognition that agents with positive inotropic activity can increase mortality despite acute hemodynamic improvement. Current guidelines from the American College of Cardiology and the American Heart Association for management of acutely decompensated CHF and decompensation of chronic CHF without cardiogenic shock advocate use of inotropic agents (dobutamine and dopamine) only if administration of morphine, loop diuretics, sublingual and intravenous nitroglycerin, and nitroprusside provide insufficient improvement. Yet, intravenous inotropic agents continue to be used commonly for this syndrome. Nesiritide is a recombinant human brain, or B-type, natriuretic peptide that is identical to the endogenous hormone produced by the ventricle in response to increased wall stress, hypertrophy, and volume overload. Nesiritide has venous, arterial, and coronary vasodilatory properties that reduce preload and afterload, increase cardiac out-

Intravenous nesiritide vs nitroglycerin for treatment of decompensated congestive heart failure: a randomized controlled trial.
CONTEXT Decompensated congestive heart failure (CHF) is the leading hospital discharge diagnosis in patients older than 65 years. OBJECTIVE To compare the efficacy and safety of intravenous nesiritide, intravenous nitroglycerin, and placebo. DESIGN, SETTING, AND PATIENTS Randomized, double-blind trial of 489 inpatients with dyspnea at rest from decompensated CHF, including 246 who received ...
متن کاملComparison of effects on left ventricular filling pressure of intravenous nesiritide and high-dose nitroglycerin in patients with decompensated heart failure.
The results of this study showed an advantage of nesiritide compared with high-dose nitroglycerin in the treatment of patients with decompensated heart failure. Nesiritide resulted in an early decrease in pulmonary capillary wedge pressure (< or =15 minutes), which was sustained throughout the study period (24 hours) without the need for up-titration. In contrast, the onset of the nitroglycerin...
متن کاملImpact of nesiritide on renal function in patients with acute decompensated heart failure and pre-existing renal dysfunction a randomized, double-blind, placebo-controlled clinical trial.
OBJECTIVES Our purpose was to evaluate the impact of nesiritide on renal function in patients with acute decompensated heart failure and baseline renal dysfunction. BACKGROUND Although nesiritide is approved for the treatment of acute decompensated heart failure, retrospective analyses have raised concerns that it may cause worsened renal function. To date, no randomized clinical trials have ...
متن کاملEfficacy and Safety of Intravenous Urapidil for Older Hypertensive Patients with Acute Heart Failure: A Multicenter Randomized Controlled Trial
PURPOSE Urapidil is putatively effective for patients with hypertension and acute heart failure, although randomized controlled trials thereon are lacking. We investigated the efficacy and safety of intravenous urapidil relative to that of nitroglycerin in older patients with hypertension and heart failure in a randomized controlled trial. MATERIALS AND METHODS Patients (>60 y) with hypertens...
متن کاملPrognosis of emergency room stabilization of decompensated congestive heart failure with high dose lasix
Objective: Congestive heart failure (CHF) has become one of the most important health care problems in western countries. This article focuses on the outpatient diagnosis and management of heart failure. We want to compare the outcome of patients who were treated with high dose diuretics in the emergency department (ED) without admission with patients who were admitted to hospi...
